Job Summary
 
This role will function as one of the key Population Health Management (PHM) subject matter experts at Health Catalyst, working with healthcare clients.
 
This role will join our Population Health Strategic Services team, a team devoted to consultative deliverables and support for our client partners, within our larger Population Health Business Unit. A major focus for this role will be on client partnership through strategic consulting, roadmap development, and operational implementation. This role will involve hands-on work with executives, providers, frontline clinical staff, and other stakeholders. This role will drive strategic consulting for our clients, leveraging assessment tools, working with Health Catalyst analysts, and synthesizing findings to make data-driven recommendations, supported by our technology.
 
The Population Health Strategic Services team supports the following service domains: Strategic Consulting, Care Management, Clinical Transformation, Contract Management Performance, Data and Analytic Consulting, Network Management, Quality Management, Performance Improvement, and Risk Adjustment Accuracy. This role will support these services, leveraging analytic and population health best practices to strategically develop tailored client strategies and operations. This individual will be expected to own one or more of the above service domains within one year of joining this team.
 
Candidates should have deep expertise in various value-based care contract models – including capitation and self-insured contracts – and population health strategies and operations. Candidates should have deep depth of experience in one of the aforementioned service domain areas. Candidates should be up to date on current trends in population health and value-based care, including alternative payment model updates, government and commercial ACO plans, and clinical and social equity conditions.
 
Specific responsibilities for this role are outlined below:
 
Population Health Management
 
Oversees overall consultative deliverable to clients
Assesses the client's current state through PHM strategic assessments, stakeholder interviews, and contract review
Oversees data-driven planning of financial, clinical, and operational improvement to support clients in value-based care strategy and decision-making
Works with analysts to produce scenario modeling and analytic intelligence for high-impact interventions, including network management, risk adjustment accuracy, and contract performance improvement initiatives that create cost-reductions or improved quality scores within value-based care models
Facilitates the design and evaluation of cross-continuum initiatives including utilization management programs, network optimization, and care management programs
Supports clients in opportunity exploration around participation in Alternative Payment Models, including capitation arrangements and health plan partnerships
Provides strategic PHM-related support across internal teammates, working with internal customers to determine product definition and business requirements
 
What you bring to this role:
 
Five years or more of population health management, data and analytics, operations, and/or quality improvement
Prior experience in consulting, clinical improvement, and/or capitation contract models
Up-to-date knowledge on population health and value-based care trends, policy changes, and impact to health systems, ACOs, and payers supporting population health
Track record of high impact in PHM data and analytic intelligence, strategy, and improvement
2+ years of consulting experience
Proficiency in evaluating appropriate combination of people, processes, and technology for population health entities or stakeholders
Ability to collect qualitative and quantitative data, conduct analyses, and synthesize results into strategic and actionable recommendations
Strong customer drive and dedication to success
Project management experience
Proven experience managing complex client relationships as a trusted consultant
The ability to ‘tell a story’ through data and visualizations
Basic understanding of Enterprise Data Warehouse tools and environment
Excellent interpersonal, presentation, written communication skills
Ability to work effectively on multiple projects in a fast-paced environment
Attention to detail, time management, prioritization, problem solving, adaptability, and willingness to learn
 
Education & Relevant Experience 
 
BS/BA in Healthcare or Analytics related field of study preferred
Advanced degree (MBA, MPH, MHA) preferred
